Tompkins Care Connection provides:

to Tompkins County senior citizens and disabled residents of any age who need long-term care.

County residents who meet program guidelines qualify for home care services through the Expanded In-home Services for the Elderly Program (EISEP) and Medicaid.
Additional services include:

  • Care at Home
  • Medicaid waivers for children
  • Medicaid Personal Emergency Response System

Tompkins Care Connection is Tompkins County's "New York Connects" program. Tompkins County residents interested in starting a family-type home (residential and non-nursing home care) for seniors can call Long Term Care Services, 274-5222, for an application.

Family Home Requirements include:
  • meeting water and fire code regulations
  • passing a safely inspection
  • applicant must:
    • be in good standing in the community
    • provide personal references
    • obtain a physician's statement of his/her ability to perform the services

The Medicaid Buy-In for Working People With Disabilities program allows people to work and have Medicaid coverage. They must be under 65; have a disability; and they must be working. DSS staff will determine eligibility. Applicants need not be certified disabled, but believe they should be. Either Social Security or DSS makes disability determinations based on the Social Security disability listings and criteria.

Title:
Division Coordinator for Emergency Services, Childcare, Fraud, Domestic Violence Liaison
Special Services provides assistance to all persons with have an emergency need for housing and/or food, or are victims of domestic violence. Special Services determines eligibility for these individuals and refers them to other services if they meet eligibility requirements. Special Services processes referrals from Red Cross to determine eligibility for emergency assistance for shelter stays. The staff also works with the Disaster Coordinator at Red Cross to help persons in natural disasters. The unit assists homeless persons to relocate and conducts domestic violence screenings. Special Services conducts home studies to determine eligibility for Public Assistance applicants under 18 who must live on their own. Special Services also administers The Housing First Program. This grant funded program offers assistance to homeless families and individuals in obtaining housing that otherwise might be unaffordable. Applicants for the Housing First Program must be screened by 2-1-1 prior to referral to Special Services. Housing First is a three- to 12-month program with intensive case management assisting the person in need to reach financial independence. DSS Special Services offers a one-time furniture voucher. If furniture is destroyed by fire, flood, etc, a further voucher can be obtained. Applicant must apply for DSS services and be determined income eligible.

Soldier On is a private nonprofit organization committed to ending veteran homelessness. Since 1994, it has been provided homeless veterans with transitional housing and supportive services.

In 2010, the organization opened the first Gordon H. Mansfield Veterans Community, a permanent housing cooperative that provides formerly homeless veterans with safe, sustainable, affordable housing, transitioning them from homelessness to home-ownership. This model will be replicated nationally as Soldier On works to eliminates homelessness among veterans.

Soldier On now serves Central NY and Tompkins and Cortland Counties. It can assist with:

  • rental payments
  • security deposits
  • child care

Eligibility:

  • discharge status other than dishonorable
  • income level 50% less than the area median
  • homeless or at risk of homelessness
Soldier On encourages veterans who THINK they are eligible to call. Soldier On will help ineligible veterans find agencies that can help.

Southern Tier AIDS Program (STAP) provides case management and supportive services to persons with HIV and their families. Supportive services include: assistance with understanding treatment, housing assistance and emergency financial assistance. STAP provides HIV Prevention Education to the community and operates specific programs for young gay/ bisexual/ transgender men, communities of color, women, incarcerated individuals and active substance users. STAP operates syringe exchange programs in Ithaca and Johnson City. Free HIV testing with results in 20 minutes is offered at the Ithaca and Johnson City offices. Free Hepatitis C testing for is offered at the Ithaca and Johnson City offices. Safer sex supplies, i.e. condoms, female condoms, lubricant, dental dams, latex gloves, are always available for free at STAP offices.

Tompkins County Prevention Point is a free syringe exchange program and harm reduction service. It prevents the spread of HIV, Hepatitis C and other infectious diseases by providing free clean IV & IM syringes and other supplies to prevent needle sharing.
