Collaborative Care Community

What is the Collaborative Care Community?

The Collaborative Care Community (CCC) has been developed by a group of over 150 educators, human service agency professionals and parents to create an interconnected network that works to increase the number of successful transitions between systems of care for children, youth and their families. Within the CCC, parents, human service agencies, hospitals and residential facilities work together, accept shared responsibility and uphold shared values. They all focus on the same goal: the success and well being of children and youth as they transition between systems of care. It takes everyone working together to create a successful transition!

What are the guiding principles of the Collaborative Care Community?

The core principles of the CCC are reflected in the Statement of Understanding that was created by over 100 stakeholders. The final Statement of Understanding reads as follows:
Whereas, all children, youth and families (in their many different forms) should have equal opportunities to live full, healthy and successful lives; and
Whereas, all children youth and families should have access to well coordinated, flexible, individualized and comprehensive mental health, educational and community support services;
We the undersigned, agree to uphold the following core values of the Collaborative Care Community to support successful transitions of children youth and families between systems of care:
  • Place the success and well-being of children youth and families at the core of our collaborative care planning on both individual and system-wide levels;
  • Embrace, in word and in action, high levels of respect and compassion for all members of the Collaborative Care Community, including children, youth, families, community partners and educators;
  • Seek and practice creative and culturally competent methods to ensure active children youth and family engagement and participation in both the Collaborative Care Teams and Collaborative Care Community;
  • Encourage proactive communication through the fostering and maintenance of relationships between stakeholders at both the front-line and administrative levels;
  • Actively engage in ongoing proactive co-education about each stakeholder’s role and function within the Collaborative Care Community;
  • Create and follow complimentary predictable linkage protocols that assure timely communication and responsibility for follow-through between systems;
  • Engage in ongoing evaluation at the system-wide level, including proactive collaborative strategic planning;
  • Engage in meaningful program-evaluation and professional self-reflection in order to improve collaborative efforts;
  • Seek feedback from children, youth, families and colleagues as a way to improve our daily practice;
  • Recognizing the urgency of many transitions, will maintain a patient, open minded and creative attitude when planning on both an individual and community wide level.
Within the CCC individualized CARE teams support successful transitions by following the Parent's Guide, the Provider's Guide and the Best Practice Checklists for Admission to Hospital or Long-term Facility and Discharge from Hospital or Long-term Faciilty.

Parent's and Provider's Guidebooks

Parent's Guide: This guide would be helpful to you if you are helping to transition your child or youth to or from home to a hospital or residential placement. This guide will help you understand how the Collaborative Care Community can support you and your child through this time. If you need more help, please use the contact information at the back of this booklet.

Service Provider's Guide: This guide would be helpful to you if you are involved in helping to transition a child or youth between systems of care. Whether you are a parent, educator, human service provider, residential care provider or hospital provider, the Collaborative Care Community will help you work in partnership, communicate effectively and contribute to the success of the young person and family you are working with. This guide should answer some of your questions and will help you see how the Collaborative Care Community works.
